Questionnaire actions for professional work
Fill in request and ask for the opinion of the workplace management where you want the survey to take place. Please submit the form with the written consent of the workplace management to the secretariat of the Deputy Minister for Non-medical Occupations and Quality - tel. +420 224 969 341.
If you want to carry out the survey in more than one workplace, you must agree to a questionnaire from each workplace. Patient research activities need to be assessed and agreed by an independent Ethics Committee (the requirement is in line with the International Ethics Guidelines for Medical Research with Human Subjects - Geneva 2002).
