Discharge from hospital

About hospital discharge You will be informed by a doctor who will also issue a discharge report to the attending physician. You will learn from the medical staff everything you need to know about the next course of treatment as well as the treatment regime at home. The dismissal message will be forwarded to you when you leave the hospital.

If your medical condition after hospital discharge requires home nursing care, it will be provided to you by appointment with you, your doctor and health and social worker VFN to the extent necessary.

Patients whose medical condition so requires ambulance transport on discharge from hospital. This is indicated by the attending physician.
